The Ontario Provincial Police are investigating the death of 34-year-old Tyrone (Tye) Lawrence Kakepetum of Sioux Lookout following a serious assault.

On August 22, 2025, police were called to a home on Millar Crescent, where Kakepetum was found with life-threatening injuries and taken to hospital. He died one week later, on August 29.

A postmortem examination was conducted in Toronto. The Sioux Lookout OPP Crime Unit, working with the OPP Criminal Investigation Branch, the Office of the Chief Coroner, and the Ontario Forensic Pathology Service, continues to investigate the circumstances surrounding his death.
