According to initial reports, around 12:30 a.m., 911 calls were made about a disturbance at a home on Cornerstone Drive in Burlington. When officers from the Halton Regional Police Service arrived, the man ran upstairs and was found with a self-inflicted injury. Police attempted life-saving efforts and called paramedics, but the man was pronounced dead at the scene.

Inside the home, another man was also found dead, and a woman was taken to hospital with non-life-threatening injuries.

The Halton Regional Police are conducting a separate criminal investigation alongside the SIU’s review. The SIU has assigned four investigators and two forensic specialists to the case. Three witness officers have been identified so far.

In the early morning of June 3, 2025, Halton Regional Police Service officers were dispatched to a residence in Burlington after having received a disconnected 911 call from the address. When the call-taker called back, they heard a conversation between a male and female suggesting the male had killed someone inside the house and was now threatening the female. This conversation was between a mother and son. Upon the arrival of officers, the mother opened the door and told the officers her son was upstairs. The son was in the ensuite bathroom of the master bedroom. An officer directed the man to drop the knife and come out. The man did not do so. When the man eventually stumbled out of the bathroom into the bedroom, an officer fired his conducted energy weapon and the man fell to the floor. Another officer handcuffed the man’s right hand and he was turned onto his back. The man was bleeding profusely from a large laceration across the neck. The officers began to administer emergency first-aid, applying pressure to the man’s wound. Paramedics and firefighters arrived on scene and took over the man’s care. He was subsequently pronounced deceased on scene. On his assessment of the evidence, SIU Director Joseph Martino determined there were no reasonable grounds to believe that any officer committed a criminal offence in connection with the man’s death.
