Hamilton Paramedic Service responded to a residence in Stoney Creek for an elderly woman experiencing medical distress in July 2025. Police said she was transported to Hamilton General Hospital, where she was admitted to the Intensive Care Unit.
Medical staff later contacted Hamilton police after raising concerns about possible elder neglect. The woman, identified as Zofija Tokic, died in hospital on July 16, 2025.
A lengthy investigation involving the Hamilton Police Homicide Unit, the Crimes Against Seniors Unit, the Ontario Forensic Pathology Service, the Centre of Forensic Sciences, and the Regional Coroner’s Office followed.
As a result of the investigation, Hamilton police announced on March 16th 2026, that Peter Tokic has been charged with manslaughter and criminal negligence causing death in connection with his mother, Zofija Tokic’s death.
Police say caregivers have a legal responsibility to provide the basic necessities of life to individuals in their care. Authorities say they remain committed to investigating cases that affect the safety and well-being of vulnerable people in the community.

Officials also remind the public that elder abuse can take many forms, including neglect, physical or emotional harm, and financial exploitation.
