Derrick Adams charged with manslaughter in relation to the 2021 fentanyl related death of Simcoe Ontario teen Rachel Cook

Norfolk County paramedics responded to a home on August 27th where they located Rachel Cook, 17, unresponsive on her bed.

Cook was taken to Norfolk General Hospital and then transferred to a Hamilton hospital where she died on Aug. 28th. Her cause of death was a fentanyl overdose.

Derrick Anthony Adams, 41, has been charged and found guilty of manslaughter and criminal negligence causing death. The Judge said “A reasonable person would have foreseen the risks of providing fentanyl,”
